- You need the UPC from the movie, and a store identified receipt for same
- You need the UPCs from the Hostess products, and a store identified receipt for same
- Mail all to address on Hostess' printable rebate form by 4/30/09 (rec'd by 5/13/09)

For the Madagascar 2 original UPC: Will one of the proof of purchase tabs suffice or do I have to cut out the actual barcode from my DVD case?
For form itself asks for "The original UPC code, neatly cut, located on the packaging of
the MADAGASCAR: ESCAPE 2 AFRICA™ DVD MOVIE"
Honestly, it says Barcode. There are proofs of purchase on printed on the paper on the inside the plastic DVD case, but it asked for the UPC and that is what I would send. I'm afraid you may need to cut your barcode off somehow.
