The promotion (not listed on its website) began 11/30/08 and runs through 2/28/09, with a post mark date of 3/14/09. I did read on Slick Deals that those who can't find the form can call the company to request one, but I can't otherwise confirm that. I did find the form in a drug store and it just appeared there within the last week or so. The form does say for Promotion questions to call 1-877-286-4800, so if you are interested and don't have a form, give them a ring since the worst that they could say is no dice.
